1) SELF-SCREENING:
PLEASE NOTE THAT AS PART OF OUR SCREENING STRATEGY, YOU NEED TO ASK YOURSELF THE FOLLOWING QUESTIONS. IF YOU ANSWER "YES" TO ANY ONE OF THESE OPTIONS, WE REQUEST THAT YOU CALL OR EMAIL US TO POSTPONE YOUR APPOINTMENT AND TO RESCHEDULE AT A LATER DATE (AT LEAST 24 HOURS AFTER SYMPTOMS HAVE SUBSIDED)
Do you have any of the following symptoms:
• Fever and/or chills
• New onset of cough or worsening chronic cough
• Shortness of breath
• Decrease or loss of sense of taste or smell
• If you are adult over 18 years of age: unexplained fatigue/ lethargy/ malaise/ muscle aches (myalgias)
• For any individual under 18 years of age: nausea/vomiting, diarrhea
2) MASKING:
We are happy to see patients whether you choose to mask or not. Our health care providers and support staff may or may not be masked during your appointment. If you feel more comfortable with your provider using a mask for your appointment, please speak to them about this.
3) TIMING AND PAPERWORK:
Please consider coming to the clinic about 10 minutes ahead of time to fill out any paperwork if this is your first consult. If you would like to fill out paperwork ahead of time, please download the "Informed Consent-physiotherapy" and "Patient Information" files below and complete and email them to us at stgeorgephysio@gmail.com
4) PATIENCE:
Downtown Toronto is a busy and congested place to live and commute in. Please give yourself enough time to get to your appointment. Parking is available but scarce (see "parking near our clinic"), and the best way to get to our facility is by taking transit or walking if you are close by and can manage. If you are running behind, please let us know and we will try our best to give you the full time you are scheduled for if possible.
